ALF Gold-Leaf Certificate of Achievement / ALF Certification Card - Duplicate

You have passed the ALF Core Competency Test.

You can request an 8”x10” Gold-Leaf Certificate of Achievement or a Duplicate of your wallet size Certification Card. Please submit the information requested below. Complete and mail the request form.

For each Certificate of Achievement order, enclose $30.00.
For each Duplicate Certification Card order, enclose $15.00.

THE AMOUNT OF CERTIFICATES OF ACHIEVEMENT OR DUPLICATE CERTIFICATION CARDS ORDERED ARE LIMITED TO THREE (3) PER EXAMINEE.

Important Notes:
Certificate processing can be delayed if your packet is incomplete or missing Continuing Education Units. You must include 12 hours of completed CEU's for every 2 year period from the date you passed the Core Examination.

New ID cards are not issued after completing continuing education requirements. Please see FAQ's for further information.
